For 25 cents, a visitor to Coney Island could walk past a row of glass boxes and watch tiny, fragile babies fight for their lives. It looked like a carnival gimmick. It was actually the best care premature babies could get anywhere in the country - and almost nobody knew the man running it wasn't a real doctor.

Written Off as "Weaklings"

In the early 1900s, most hospitals had no equipment and little interest in premature babies. They were commonly labeled "weaklings" and sent home to die, since doctors believed there was nothing to be done for a baby born months too early. Martin Couney disagreed. Starting in 1903, he installed rows of incubators - steel-walled boxes with glass fronts, heated by water boilers running underneath - inside a sideshow attraction at Luna Park. In 1904 he opened a second exhibit at the neighboring Dreamland amusement park.

25 Cents a Ticket, Free Care for Parents

Couney charged the public 25 cents to view the incubators, and he ran a full medical operation behind the glass: wet-nurses, trained attendants, and round-the-clock monitoring. The setup cost roughly $15 a day per baby in 1903, all of it paid for by admission tickets. Parents were never billed a cent. Babies came to him directly from local maternity wards that had no way to treat them.

The Night Dreamland Burned

In 1911, a fire tore through Dreamland and destroyed the amusement park. Every baby inside Couney's incubator exhibit made it out safely, and care continued without interruption at his other exhibit at Luna Park, where he kept the incubators running until 1943.

A Showman, Not a Doctor

Here is the detail that makes the whole story stranger: Couney, born Michael Cohn in Prussia, never had a documented medical license in the United States. He styled himself "Dr. Couney" for decades, and no record has ever surfaced of him completing formal medical training recognized by American authorities. Yet his methods worked. By the time he closed his exhibit in 1943, roughly 6,500 of the 8,000 babies in his care had gone home, and mainstream hospitals had finally begun adopting the same incubator care he'd been running as a sideshow for 40 years.

What Finally Changed

Couney spent decades trying to get hospitals to take premature infant care seriously, and for most of his career they simply refused. It took his sideshow, not a medical journal, to prove incubator care worked at scale. By the 1940s, hospitals across the country had built their own incubator wards - putting the Coney Island attraction that started it all quietly out of business.
